PAYROLL DEDUCTION AUTHORIZATION 

I, the undersigned employee of Reward Transportation Services, LLC, hereby authorize the Company to make the following deductions from my paycheck to the extent permitted by applicable Pennsylvania and federal law:

AUTHORIZED DEDUCTION TYPES

• Repair or replacement costs for Company vehicle or equipment damage caused by my negligence or policy violation

• Costs associated with uniforms, equipment, or Company property issued to me and not returned upon separation

• Overpayment recovery — in the event of a payroll overpayment

I understand and agree to the following: 

• Deductions will not reduce my pay below the applicable minimum wage for hours worked in a given workweek, in compliance with the Pennsylvania Minimum Wage Act and the Fair Labor Standards Act. 

• This authorization is not a waiver of any claims I may have and does not constitute an admission of liability on my part. 

• A copy of this signed authorization will be placed in my personnel file.
