I acknowledge that I have received and understood training on each of the topics checked above, and that I have completed the annual competency evaluation indicated. I understand this training is required at least annually and that I am responsible for maintaining the knowledge and skills covered.

I acknowledge that I have received access to the Reward Home Care Employee Handbook and understand that it is my responsibility to read, understand, and comply with all company policies, procedures, safety requirements, attendance expectations, standards of conduct, and job responsibilities.

I understand that the handbook is a general guide and does not create an employment contract or guarantee employment for any specific period. I further understand that Reward Home Care may update its policies as needed, and that violations may result in corrective action, up to and including termination.
