Reward Home Care requires all employees who operate Company vehicles to hold a valid driver's license and maintain an acceptable Motor Vehicle Record (MVR). An MVR check will be conducted prior to employment and may be repeated periodically during employment.
MVR STANDARDS — DISQUALIFYING EVENTS
EMPLOYEE AUTHORIZATION
By signing below, I authorize Reward Home Care, and any designated background/MVR screening vendor, to obtain a copy of my Motor Vehicle Record for the purposes of evaluating my eligibility for employment and continued assignment to vehicle operations. I certify that the driver's license information provided above is accurate and complete. I agree to immediately notify the Company of any changes to my license status, suspensions, revocations, or new traffic violations.
The following events on an employee's driving record may disqualify an individual from employment or vehicle operation privileges:
DUI, DWI, or any alcohol/drug-related driving conviction within the past 5 years
Reckless driving conviction within the past 3 years
More than 2 at-fault accidents within the past 3 years
More than 3 moving violations within the past 3 years
License suspension or revocation within the past 3 years
Leaving the scene of an accident (hit and run)
Driving with a suspended or revoked license
Any felony conviction involving a motor vehicle
I also agree to notify the Company within 24 hours of any of the following:
Receiving a traffic citation or violation while operating a Company vehicle
Being involved in any motor vehicle accident — on or off duty
Any change to my license status (suspension, restriction, expiration)
